Description
This personal wellbeing reflection and feedback form was created to help participants at Women’s Wellness Day reflect on what they would like to get out of the day and what strategies they can do to improve their own health and wellbeing, as well as what they might light to speak to their therapist about.
The second half of the form provides us with feedback about the day's program and assists with our evaluation. The Rainbow Beach event was the first time we used and it was well worth it.
